Pregnancy is an optional kink, which is turned off by default and must be manually enabled in the Sanctum. It can affect both the player and some characters.
Note that enabling pregnancy, getting pregnant (either yourself, or impregnating the others), and then turning off the pregnancy would not terminate the ongoing pregnancies.

There's a 10% chance of being impregnated after any scene involving vaginal creampie (assuming Lust Doll has a vagina). A Fertility Pill raises it to 100% for any scene. Note that one pill ensures only one pregnancy, and must be taken again to reapply the effect after giving birth.
If impregnation occurs successfully, the player gets notified about getting the Pregnant: Early Stage effect. The early stage has no inherent effects on its own, but after some time it progresses into Pregnant: Late Stage. The late stage directly affects stats, adds a visible belly to Lust Doll's portrait, and modifies some encounters.
The late stage pregnancy never wears off on its own; the only way to give birth is to visit the Nursery in New Ark City.
Some encounters have different chance of pregnancy than normal:
- Slime Monster: has 100% chance to trigger on "Slime immerse (Pussy)" scene. Though this pregnancy is different, too.
- If Cumflation is enabled, intercourse with the Infected Scientist has guaranteed pregnancy, due to vastly increased amount of cum pumped into Lust Doll.
Some named characters, if they manage to impregnate the player, have special lines for it:
NPC pregnancy[edit | edit source]
Some characters may be impregnated by the player. Unlike the player, those characters are required to take a Fertility Pill to get pregnant, which can be gifted to them; it impossible to impregnate them until certain story progression with them gets achieved.
Their pregnancy varies in time needed to develop, with some taking longer than the others. While in the late stage, their appearance changes, and additional scenes become available.
